Coach Kit Letter

Dear Coach:

Please review the additional information in your profile under Checklists/Kits to ensure you have a complete understanding of your role as Coach during your date of participation at Cooperstown Dreams Park.

Cooperstown Dreams Park is a children's camp licensed by the State of New York Department of Health. We are required to outline the rights and responsibilities of all campers and camp operators.

  1. Cooperstown Dreams Park is required to be permitted to operate by the New York State Department of Health (Chapter I State Sanitary Code, Subpart 7-2, Children's Camps).
  2. Cooperstown Dreams Park is required to be inspected twice yearly.
  3. All inspection reports and governing laws concerning Cooperstown Dreams Park are filed at the New York State Department of Health, Oneonta District Office, 28 Hill Street, Suite 201, Oneonta, New York 13820.

Rights of Parents and Guardians:

  • To be informed by the Camp Director, or his or her designee, of any incident involving your child, including serious injury, illness or abuse.
  • To review inspection and investigation reports for a camp, which are maintained by the local health department issuing the camp permit to operate (present and past reports are available).
  • To review the required written camp plans. These are on file at both the camp and the health department issuing the permit to operate.
  • To remove or to sign-out any camper at any time they deem appropriate provided they notify the Camp Director or follow the Check-Out procedure as established by Cooperstown Dreams Park.

Special attention, emphasis and review should be placed on the Safety, Training and Supervision (STS) Guide which is in your profile under Checklists/Kits. You should review the STS Guide multiple times so you completely understand your role and obligations to the campers and to Cooperstown Dreams Park. You will be required to go through an STS training session as part of the registration process.

We suggest at a designated team meeting, with all team members' parents present, that you review your role, the parent and camper roles, and review and execute all forms.

You are encouraged to use the Team Contact/Coach Checklist to ensure you arrive at Cooperstown Dreams Park with all the necessary documents and forms. Upon confirmation by Cooperstown Dreams Park staff that all necessary documents, forms and registration are complete your team then will then qualify to participate in your tournament date.
